**INFORMATION ABOUT THE CONDITION REPORT (COMMITTEE REPORT) TO BE OBTAINED FROM A PRIVATE HOSPITAL:

In accordance with the Guidelines on the Procedures and Principles for Health Reports, in order to obtain permission to issue a Medical Condition Report, private healthcare providers must have the following specialties registered in their hospital license: "Internal Medicine, Otolaryngology, General Surgery, Ophthalmology, Orthopedics and Traumatology, or Physical Therapy and Rehabilitation, Neurology, or Psychiatry and Mental Health" must be registered in the hospital license, and a physician specializing in this field must work full-time at the hospital.

In this regard, it is important that the procedures be carried out in accordance with the guidelines when obtaining a status report (committee report) from a private hospital to prevent our dear students from being disadvantaged.
